I attach my first attempt at creating an index to manage the extensive listing of issues for which we are developing solutions.
Ken - I have used Excel, as I have never liked the Word table feature, but can export to Word, if required. I am thinking the articles be hyper-linked to the list of issues, is this feasible, or can you suggest a better delivery method?. Do you require any specific format for going live with the index?
Thought/comments for additions/deletions/changes are appreciated.

I can't open this on my iPad. We are in transit in AZ for the next couple of days but I will keep trying when I get an opportunity.

I believe this is good in order to organize things. When it comes to publishing the articles on the website the index will automatically be created based on the category and title of the documents.
As an example you can see how the Manuals appear, the technical docs can be organized in the same way online.

I'm using the new Excel 2016, so can try saving it in an older format. Will re-post another version.

I was able to open with a little manipulation...If everyone can "Save As" an older version of the software (i.e. Word, Excel 2002), many more folks may be able to open it and comment.

I have subsequently uploaded a copy in both .xls and .pdf formats, so everyone should be able to access.
